يتيح الموقع طلب اتصال وتنسيق خدمات المركز. الحساب اختياري، ويجب تقديم بيانات اتصال صحيحة ومراجعة الرسائل. إذا كنت تساعد مريضاً، فيجب أن تكون مخولاً بتقديم معلوماته؛ يراجع الفريق التفويض قبل إتاحة معلومات الملف. حافظ على سرية رموز الدخول ولا تشاركها.

تأكيد المواعيد والأسعار

استلام الطلب أو اختيار تاريخ مفضل ليس تأكيداً للموعد. يتحقق الفريق من التغطية والتوفر والخدمة والسعر النهائي والضريبة عند انطباقها. تحدد تفاصيل الباقات وعدد الجلسات والصلاحية وسياسة التأجيل والإلغاء قبل التأكيد. لا نضيف هنا رسوماً أو غرامات لم يتفق عليها معك.

المعلومات الطبية وحدود النموذج

محتوى الموقع للتعريف بالخدمات وليس تشخيصاً أو خطة علاج شخصية، ولا يضمن نتيجة علاجية معينة. نموذج الاتصال والتقييم ليسا قناة للطوارئ أو للإرشاد الطبي العاجل. في الحالات الطارئة تواصل مع خدمات الطوارئ المحلية. أرسل المعلومات الطبية فقط عبر القنوات التي يحددها فريق الرعاية.

The website supports callback requests and coordination of center services. Accounts are optional. Provide accurate contact details and review messages. If assisting a patient, you must have appropriate authority to supply their information; staff review authorization before file information is made available. Keep sign-in codes private.

Appointment and price confirmation

Receipt of a request or selecting a preferred date is not appointment confirmation. Staff confirm coverage, availability, service, final price and applicable tax. Package inclusions, session counts, validity and postponement/cancellation terms are explained before confirmation. This page does not introduce fees or penalties that have not been agreed with you.

Medical information and form limits

Website content describes services and is not a diagnosis or an individual treatment plan; it does not guarantee a particular treatment result. Callback and feedback forms are not emergency or urgent medical-advice channels. For an emergency contact local emergency services. Share medical information only through channels designated by the care team.

Email, phone and communication choices

Email and phone support sign-in, responses, coordination and necessary follow-up. With separate optional permission they may also be used for offers through email, calls, SMS and WhatsApp. Marketing permission is not a condition of care or an account and can be withdrawn without affecting service.
