مركز الفزعة للرعاية الطبية المنزلية يقدم هذا الموقع لتنسيق خدماته داخل المدينة المنورة. للتواصل: info@fazatnacare.com أو 0556506934. لا يطلب نموذج الاتصال الأول تشخيصاً أو رقم هوية أو نتائج طبية.

ما نجمعه ولماذا

نستخدم اسم المتصل والجوال والخدمة المطلوبة والحي والتفضيلات الاختيارية للاتصال والتحقق من التغطية والتوفر. الحساب اختياري؛ نستخدم البريد ورمزاً مؤقتاً للدخول. لا يثبت رمز البريد هوية المريض أو ملكية الجوال، ولا يمنح الوصول إلى السجلات الطبية. يستكمل الملف بعد تأكيد الموعد ومراجعة تفويض المريض أو المرافق.

رسائل الخدمة والتقييم

قد نرسل رسائل الدخول وتأكيد الطلب وتغييرات الموعد والمعلومات اللازمة لتنسيق الخدمة. عند اختيار بريد التقييم ومراجعة عنوانك، يمكن إرسال دعوة خاصة بعد اكتمال الزيارة، بحد أقصى دعوة واحدة في اليوم للبريد نفسه. التقييم اختياري ولا يؤثر على الرعاية، ولا ينشر على منصات عامة. لا تكتب تشخيصات أو أرقام هوية أو معلومات طبية في التقييم. يمكنك إيقاف دعوات التقييم من رابط الرسالة.

التسويق اختيار منفصل

إذا وافقت اختيارياً، يمكن استخدام البريد والجوال لإبلاغك بعروض المركز وأخباره عبر البريد والمكالمات والرسائل النصية وواتساب. الاختيار غير محدد مسبقاً، وليس شرطاً لإنشاء حساب أو الحجز أو تلقي الرعاية. إنشاء الحساب أو طلب رمز الدخول لا يشترك في التسويق تلقائياً. يمكنك سحب الموافقة بالاتصال أو مراسلة info@fazatnacare.com دون التأثير على الخدمة. لا نبيع بيانات المرضى أو نستخدم البيانات الصحية الحساسة لصنع قوائم إعلانية.

المشاركة والاستضافة والخدمات الخارجية

يصل الموظفون المخولون إلى البيانات حسب صلاحياتهم. نستخدم مزودي استضافة وبريد للتشغيل؛ استضافة نظام المركز حالياً في فرانكفورت، ألمانيا. قد تعالج هذه الخدمات بيانات تقنية خارج المملكة. اختيار واتساب يفتح خدمة خارجية ولا يرسل الرسالة تلقائياً. لا تتضمن رسائلنا تشخيصات أو مرفقات طبية. لا يستخدم مسار التسجيل بكسلات إعلان أو إعادة تشغيل جلسات، ولا نرسل بيانات المرضى أو الهواتف أو النتائج القابلة للتعريف إلى منصات الإعلان. أي أدوات أو حملات مستقبلية تحتاج مراجعة مستقلة للبيانات والموافقات وقواعد المنصة.

الحماية ومدة الاحتفاظ

نستخدم اتصالات مشفرة وصلاحيات محددة وسجلات تشغيل للمساعدة على حماية البيانات. رمز الدخول صالح لمدة 10 دقائق ورابط التقييم صالح لمدة 30 يوماً. نحتفظ بالبيانات بالقدر اللازم للتنسيق والمتابعة والمتطلبات النظامية؛ قد تختلف مدة حفظ السجلات الطبية والمالية عن طلبات الاتصال. لا يعني حذف حساب أو سحب التسويق حذف سجلات يلزم حفظها نظامياً. لا يعد الموقع بضمان أمان مطلق.

الجلسات وحقوقك

يستخدم الحجز ملفات جلسة ضرورية للحماية والدخول. يحفظ المتصفح تفضيل اللغة؛ المسودات على الخادم ولا تحفظ معلومات الملف الطبي في localStorage. نقيس داخلياً أعداد التفاعل والطلبات إجمالياً. يمكنك التواصل بشأن الاطلاع على بياناتك أو تصحيحها أو طلب حذفها أو سحب الموافقة. قد نتحقق من التفويض قبل الاستجابة، ونراجع الطلب وفق الالتزامات المطبقة.

Who we are and purpose

AL FAZAA Home Healthcare Center uses this website to coordinate its services within Madinah city. Contact info@fazatnacare.com or 0556506934. The initial callback form does not request diagnoses, identity numbers or medical results.

What we collect and why

We use contact name, phone, requested service, neighborhood and optional preferences to respond and check coverage and availability. Accounts are optional; email and a temporary code support sign-in. Email verification does not prove patient identity or phone ownership and does not grant medical-record access. Patient-file completion follows appointment confirmation and appropriate patient/caregiver authorization checks.

Service messages and feedback

We may send sign-in, request confirmation, appointment-change and essential coordination messages. With a separate feedback choice and confirmed address, a private invitation may follow a completed visit, limited to one invitation per email per day. Feedback is optional, does not affect care and is not published on public review platforms. Do not include diagnoses, identity numbers or medical information in feedback. Stop feedback invitations from the email link.

Marketing is a separate choice

If you separately opt in, email and phone may be used for center offers and news through email, calls, SMS and WhatsApp. The choice is unchecked and is not a condition of an account, booking or care. Account creation or requesting a code does not automatically subscribe you to marketing. Withdraw by calling or emailing info@fazatnacare.com without affecting service. We do not sell patient information or use sensitive health data to build advertising audiences.

Sharing, hosting and external services

Authorized staff access information according to their permissions. Hosting and email providers support operation; the clinic system is currently hosted in Frankfurt, Germany. These services may process technical data outside Saudi Arabia. WhatsApp opens an external service and does not automatically send a message. Our emails contain no diagnoses or medical attachments. Registration uses no advertising pixels or session replay; patient information, phones and identifiable outcomes are not sent to advertising platforms. Future tools or campaigns require a separate review of data, consent and platform rules.

Protection and retention

Encrypted connections, access restrictions and operational records help protect information. Sign-in codes expire after 10 minutes and feedback links after 30 days. Information is kept as needed for coordination, follow-up and applicable obligations; medical and financial records may have different retention requirements from callback requests. Account deletion or withdrawing marketing does not necessarily remove records that must be retained. No website can promise absolute security.

Sessions and your choices

Booking uses necessary session cookies for protection and sign-in. The browser stores a language preference; drafts stay on the server and medical-file information is not stored in localStorage. Internal aggregate counts measure interactions and requests. Contact us for access, correction, deletion requests or consent withdrawal. We may verify authority before responding and review requests against applicable obligations.
